We have attached an example of a standard subpoena asking a police officer to appear at a hearing.  Would you please note that you serve all subpoenae for personal appearance(s) of Chicago Police Department Personnel on:

We have found it advisable to bring at least two copies of the subpoena to Superintendent Rice's Office together with the original.  Also, if your subpoena is defective in any way, the officer on duty will not accept service.
