1. We have attached an example of a standard subpoena to obtain records from the Chicago Police Department in order to show you the format. Please note to whom the subpoena is addressed, the language used to request records, and the types of information needed to identify the records you are seeking.

2. Make at least two copies of the original Subpoena, at least one copy will be served on the Keeper of the Records and one copy will be your file copy. (The original of any Subpoena is kept in our file in the event that requested materials aren't delivered. Then, the Court will need the original Subpoena to order that the requested materials be delivered.)

3. The Subpoena should be served on:

4. You should obtain the name of the officer (or clerk) on whom you have served the Subpoena. (Upon your return to the office you will have to fill out the back of the Subpoena including the name of the person who was served and the date, and you will have to sign the proof of service and have it notarized. Please note that witness and mileage fees generally do not apply to criminal cases, and therefore, this part of the form usually can be ignored.)

5. The officer (or clerk) on duty should:

        a. Keep at least one copy of the Subpoena and

        b. Time stamp the original and your file copy

6. Typically, when precessing the Subpoena, the Records Department will make sure that the subpoena materials are delivered to the correct courtroom before the hearing date.

7. If you wish to make general inquiries of the Records Department, phone the Chicago Police Department at 744-6262.

8. If you with to check the subpoena log to find out if materials have been delivered to court, phone the Chicago Police Department at 744-6367 with particulars from the Subpoena so that they can identify the materials in question.
