How to Complete a Private Education Loan Applicant Self-Certification form
You will obtain this form from your lender. It is also available in our office. This contains details about federal loan availability and encourages you to seek out federal loans before applying for federal loans. This form must be completed and signed by the applicant and returned to the lender before the loan can be disbursed. You have 180 days to complete this form back and send it back to the lender, or the loan will be cancelled. If you need help in completing the form please contact our office.
Section 2: Cost of Attendance and Estimated Financial Assistance
A: Cost of Attendance for the period enrollment of the loan. This can be obtained from the myIIT. portal if you are receiving federal financial aid. If you do not have a federal financial aid award or cost of attendance information on the myIIT portal please contact our office.
B: Estimated Financial Aid. Please indicate the scholarship and loan amounts that you plan to take accept that are listed on your Financial Aid award. You an access your financial aid award via the myIIT portal.
C: Difference between Cost of Attendance and Estimated Financial Aid. This can be obtained by subtracting your Cost of Attendance from the Estimated Financial Aid.
Section 3: Applicant Information
Enter your personal information and contact information. Enter the loan period covered by the loan. The Loan period for the academic year is August through May of the previous year. For the summer term the loan periods is from May to July.
Section 4: Sign and date form
Return the form within 180 days or the loan will be cancelled by the lender and you will need to reapply.
