Update: How to Get a Replacement U-Pass
Effective October 4, 2004, CTA is moving its headquarters out of the Merchandise Mart. If you need a replacement U-Pass, you will need to go to the new CTA Headquarters at 567 W. Lake Street, 2nd Floor on Monday - Friday between 8:00am - 4:30pm. You can find answers to all of your questions at the U-Pass Information site.

Health Insurance Cards
If you enrolled in the student health insurance plan, you should have received your insurance card. If your card has not arrived, please contact The Chickering Group customer service at (800) 841-3140 to request a card. Please Note: For those students who enrolled during the extended deadline period, you should receive your insurance cards by October 11, 2004. The Health Insurance FAQ contains additional information about seeing a doctor or making a claim prior to having an insurance card.
