Video of "Outlining Course
Materials" Workshop
Did you miss last week's outlining program? If so, you can
watch it on video. Video tapes are available in the Office
of Student Services, room 310A.
Health Insurance Cards
If you enrolled in the student health insurance
plan and have not yet received an insurance card, you should
receive your card by the end of the week. If your card does
not arrive by the beginning of next week, please contact
The Chickering Group
customer service at (800) 841-3140 to request a card.
If you don’t have an insurance card yet and need to see
a doctor or have a prescription filled, you have a few options.
First, depending on what is wrong, you can use the Student
Health Center at IIT, where you don't need the insurance
information. If you prefer to see your own doctor, you will
have to pay out-of-pocket and get reimbursed from Chickering.
Students needing to have prescriptions filled will also
need to pay out-of-pocket and submit a claim form. (See
the Student
Health Insurance FAQ for links to claim forms.) Alternatively,
some doctors might process your claim without an insurance
card if you explain that you have student insurance and
are waiting for your card. You will need to provide the
doctor with IIT's policy number, 724532, and your ID number,
which is your social security number. Temporary insurance
cards with this information are available in the Student
Health Insurance Plan brochure that was mailed to your homes
during the summer. You can also pick up a copy of this brochure
at the Registar's Office.
